Claim Form Dental Claim Form: To access a dental claim form, enrollees can log into Member Portal. They can also call Customer Service at (877) 395-9420 or send an email to customerservice@deltadentalnm.com to request a dental claim form.

Enrollment/Coverage Status Form (paper version) A printable version of Delta Dental of New Mexico’s standard form for enrolling eligible employees and dependents. Can also be used to make changes to enrollee information. This version needs to be printed and filled out by hand.
Enrollment/Coverage Status Form (electronic version) An electronic version of the enrollment/coverage status form. This version can be filled out electronically, except for the signature and date. Please print it, sign it, and then return a paper or scanned copy to Delta Dental of New Mexico.
